If you are a victim of Identity Theft, and live within the city limits of Poulsbo, you can use this form to report it.
Vacation Check
If you are going on vacation, live within the city limits of Poulsbo, and would like to have our Citizen Volunteers check on your house while you are away, you can fill out the Vacation Check form.
Victim Medical Release
If you are a victim of a crime under current investigation by the City of Poulsbo, you can fill out the Medical Release Form to grant the Poulsbo Police Department access to your Medical Records in relation to the case.
Voluntary Statement
