The operator or owner of any vehicle invoved in a collision within this state in which any person is injured, including one’s self, or in which any person’s property sustains damages in the amount of $1,000.00 or more is required to complete this collision report form.

Request for Notification of Firearm(s) Return

To request the Poulsbo Police Department notify you when a family or household member will have a firearm(s) returned, complete Notification of Firearm Return and deliver it to the Poulsbo Police Department as outlined herein. The PPD will provide notice within one business day to you the requirements for release have been met and the individual is requesting release of the firearm(s). The firearm(s) will be held for 72 hours after notification is sent and then released.  Please note that the agency considers notification to be made when the message is sent using the method(s) prescribed herein.

In order to qualify for notification of a firearm(s) return, you must be a family or household member of the individual from whom the firearm(s) was taken.  RCW 26.50.010 defines a family or household member as:

  • Spouses or former spouses;
  • Domestic partners or former domestic partners;
  • Persons who have a child in common regardless of whether they have been married or have lived together at any time;
  • Adult persons related by blood or marriage;
  • Adult persons who are presently residing together or who have resided together in the past;
  • Persons sixteen years of age or older who are presently residing together or who have resided together in the past and who have or have had a dating relationship;
  • Persons sixteen years of age or older with whom a person sixteen years of age or older has or has had a dating relationship; and
  • Persons who have a biological or legal parent-child relationship, including stepparents and stepchildren and grandparents and grandchildren

Concealed Pistol Licenses

NEW APPLICATIONS – Fingerprinting is required in order to process a NEW Concealed Pistol License (CPL) Application. Fingerprinting for new CPLs is completed by appointment only. Appointments are available every Tuesday from 9am-11am and 2pm-3pm. You may call the Poulsbo Police Department administrative offices at 360-779-3113 to schedule your appointment. 

  1. New CPL applicants must reside within the city limits of Poulsbo
  2. Applicants must submit a completed Concealed Pistol License Application
  3. Applicants must submit a copy of their current driver’s license
  4.  Applicants must be fingerprinted (see notes above regarding scheduling)
  5. The fee for a new Concealed Pistol License is $49.25 which can be paid by cash, check, or credit

RENEWALS – The Poulsbo Police Department is currently processing renewals via mail or drop off and the procedures are as follows:

  1. Renewal applicants must reside within the city limits of Poulsbo
  2. Applicants must submit a completed CPL Application for renewal
  3. Applicants must submit a copy of their current driver’s license
  4. Applicants must submit a copy of their current CPL (if available)
  5. Indicate the return address desired for the CPL Renewal
  6. The application must be received within 90 days prior to the expiration date or within 90 days after the expiration date of the CPL
  7. Please enclose a check for the fees, made payable to the City of Poulsbo:
    • Renewal License = $32.00 (application received within 90 days prior to expiration)
    • Late Renewal = $42.00 (application received within 90 days following current CPL expiration)
  8. Submit application, license copies and check via mail to:

Welfare Check

If you are concerned for the well being of an individual and are able to provide a physical address (located in the city limits of Poulsbo), the Poulsbo Police Department is able to complete a welfare check. This will mean an officer being dispatched to the location of the individual and making contact with the person in question. In order to request a welfare check please contact 911 and an officer will be dispatched to contact you. If you live outside of Kitsap County please call 360-308-5400 to reach our dispatch center.
